Acknowledgment Overview

In order to make sure you have reviewed and received the information found on this website, you must complete an Acknowledgement form. This form must be completed before any settlement funds can be disbursed to you.

We use an e-Signature service called SignNow that allows you to electronically sign the Acknowledgment. The form pre-fills with the name “Guest Signer” and the initials “GS.” Please make sure to delete “Guest Signer” and “GS” and replace it with your name and initials.

We will send you a copy of your signed Acknowledgment via email within 2-3 business days. Please keep a copy for your records.

You only need to complete the acknowledgment one time. However, we recommend that you bookmark this website before proceeding so that you can return to view the information if/when you receive money from the settlement.
